Building an Instrument Reprocessing Kit: The Complete Checklist

An instrument reprocessing kit is not one product — it is seven stations, each of which stops the workflow cold if it is out of stock: point-of-use presoak, cleaning, ultrasonic, inspection and lubrication, high-level disinfection or sterilization, packaging, and monitoring plus documentation. This checklist lists what each station needs, what decides the specification, and which items are consumables you will reorder. Print it, walk your decontamination area with it, and order against the gaps.

First, classify the device — it decides the whole path
The Spaulding classification used throughout the CDC guideline for disinfection and sterilization sorts every device into one of three risk tiers, and the tier decides which stations you need:
- Critical — enters sterile tissue or the vascular system (surgical instruments, implants). Requires sterilization.
- Semi-critical — contacts mucous membranes or non-intact skin (endoscopes, laryngoscope blades, specula). Requires at minimum high-level disinfection.
- Non-critical — contacts intact skin only (blood pressure cuffs, stethoscopes). Requires low- or intermediate-level disinfection.
A clinic reprocessing only semi-critical devices needs stations 1–5 and can skip packaging and sterilization monitoring. A surgical facility needs all seven. Everyone needs 1–4, because cleaning is universal.

Station 1 — Point of use and transport
Soil that dries onto an instrument is dramatically harder to remove, and dried protein is what survives into later stages. A single-dose presoak such as EZ-Zyme multi-enzymatic presoak starts enzymatic action at the point of use or in transport, which cuts scrubbing time at the sink and protects lumened instruments you cannot easily rescue later. This is the cheapest station in the kit and the one most often skipped.
Station 2 — Manual cleaning
Two consumables, and both are required. The chemistry digests soil: a multi-enzyme detergent such as the McKesson multi-enzymatic instrument detergent combines protease, lipase, and amylase for mixed surgical, GI, and dental soils. The brush supplies the mechanical action the chemistry cannot reach — inside box locks, serrations, and lumens. Stock three types: a 7-inch nylon handle brush for general surfaces, a cannula brush sized to your narrowest lumen, and a long flexible channel brush if you reprocess scopes. Station 3 — Ultrasonic
Ultrasonic cleaning reaches geometry a brush cannot, but only if the solution is low-foaming — excess suds absorb the energy and damp out the cavitation that does the work. Use a solution specified for ultrasonic use, such as the MADA general purpose ultrasonic cleaner, rather than decanting whatever concentrate is at the sink.
Station 4 — Inspection and lubrication
Inspect every instrument for residual soil and damage under magnification before it goes any further; this is the last point at which a defect is cheap to catch. Then lubricate hinged instruments with a water-soluble, steam-permeable product such as the McKesson instrument lubricant and rust inhibitor — never an oil or grease, which leaves a film steam cannot penetrate. Station 5 — High-level disinfection (semi-critical devices)
Three items, not one, and the second is the one that gets forgotten at reorder time:
- The HLD solution itself — glutaraldehyde, OPA, or hydrogen peroxide. Our high-level disinfectant buyer's guide compares the chemistries.

Station 6 — Packaging (critical devices)
Packaging has to match the sterilization method. Steriking sterilization pouches are rated for EO gas and steam; McKesson sterilization wrap covers tray sets, with dual-layer SMS wraps available for steam, EO gas, and hydrogen peroxide. Add steam indicator tape to secure and mark packs, and load labels so any pack can be traced back to its cycle.
An indicator on the outside of a pack tells you the pack was processed — not that the contents were sterilized. That distinction is what station 7 exists for.

Station 7 — Monitoring and documentation
Two independent layers. Chemical indicator strips go inside packs to confirm the sterilant reached the contents in that load. Biological indicators challenge the cycle with live spores and are the only monitor that demonstrates lethality; run them on your facility's defined schedule and with every implant load. Staff protection at every station

The four items that stop a workflow
From the reorder pattern, these are the consumables that run out first and halt everything downstream: enzymatic detergent, brushes, MEC test strips, and chemical indicators. Detergent and brushes are obvious. Test strips are the sleeper — a department will hold three months of HLD solution and one bottle of strips, and when the strips expire on open-life the solution cannot legally be used.
