How do I size a tube brush?

Size to the bore, not to the tool. The brush should be the inside diameter of the tube or slightly larger so the fill makes contact all the way round; an undersized brush polishes a stripe and leaves the rest.

What is a CrossFlex or abrasive-nylon bore brush for?

Abrasive nylon carries grit in the filament, so it deburrs and finishes the bore without the gouging a wire brush would cause. The grit number works the same way as any abrasive: lower cuts more, higher finishes finer.

Which fill for a heat-exchanger or condenser tube?

Match the fill to the tube. Brass or nylon on copper and brass tubes, which a steel brush would score; stainless on stainless, so nothing is left behind to rust.
